St. Ambrose Sports Medicine Departmental Mission Statement
Aligned with the Mission statements of St. Ambrose University and ORA Orthopedics, the Sports Medicine program looks to provide state-of-the-art methods for the prevention of injuries; outstanding care, treatment and rehabilitation of injuries; and to provide educational opportunities to athletes and the physically active people throughout the Quad Cities.

Standard Operating Procedures


Athlete Pre-Participation Paperwork
Every athlete must fill out mandatory online paperwork through our on-line portal using ATS. Every first year athlete at St. Ambrose University must complete the pre-participation paperwork, turn in an up to date physical, and show proof of primary insurance coverage before they can compete in any team related activities. The following link will guide your through the process of completing the on-line paper work.
